Blepharoplasty involves removing excess skin from either the upper or the lower eyelids, together with any superfluous muscle. This involves restoring and tightening connections between the eyelid skin and the eye opening muscles, which have stretched out over the years. Protruding fat from the upper and lower lids often requires either removal or restraining.
Eyelid operations are often independent procedures. Occasionally just the upper lids or only the lower lids need surgery. Sometimes only fat is removed. Commonly a simple brow lift suffices and does away with the need for upper lid surgery, other times creation of neat and precise upper lid folds by anchoring eyelid skin to deeper structures is necessary. A combination operation, corono-canthopexy, is usually recommended to enhance and improve the overall outcome with the lower lid surgery. The corono-canthopexy is a frontal lift (brow lift) combined with lower lid tightening. This combined procedure restores tone, tilt, and a youthful almond shape to the lower lids and aperture, something blepharoplasty alone rarely does unless corono-canthopexy accompanies.
